This is my Case...

A cause of the unexpected failed airway
Reason to think of why pt is difficult or almost impossible to bag up with a BVM bag valve manual resuscitator. Reason to think about why the LMA failed you other than incorrect use of an LMA like device. Or reason why direct laryngoscopy without a bougie fails you.
