Thrombolytics more important than intubation of the patient with hypotension and hypoxia. Fluid bolus, iv pressors, central line get that in. Heparin ivss while philosophying when and why to treat the massive p.e. with Thrombolytics. Resuscitate the patient first. More important that intubation.

Fear TPA? It's given like water for strokes by neurologists. Why not give it to a truer, more hemodynamic emergency?
